IRS Form 1095-C Availability Notice
In accordance with IRS regulations, LifeROOTS, Inc. provides Form 1095-C (Employer Provided Health Insurance Offer and Coverage) to employees as required under the Affordable Care Act. Although forms may be distributed automatically, employees and former employees may also request a copy of their 2025 Form 1095-C at any time through October 15, 2026.

Carol Epstein

Professional Background:
Carol Epstein brings 40 years of experience in the financial industry, having held various roles in branch banking, from teller to branch manager. In the last 15 years of her career, she specialized in operations and compliance, ensuring adherence to policies and procedures.

Education:
Graduate of CBA Executive Banking School.

Involvement with LifeROOTS:
As a parent of a past LifeROOTS participant, Carol joined the board to support the organization’s mission of promoting independence and providing essential services to underserved and special needs communities.

Caregiver Coaching model

Caregiver Coaching model – is an approach in early intervention (EI) that equips caregivers with the knowledge, skills, and strategies to support their child’s learning and development through everyday routines and interactions. By collaborating with EI providers, caregivers gain confidence and competence in addressing their child’s unique needs.

This model works best because it empowers caregivers—those most involved in a child’s daily life—to integrate learning opportunities into regular routines, fostering consistent and meaningful development. It focuses on individualized guidance, problem-solving, and reflective practices, ensuring families feel supported and capable of facilitating their child’s progress.

Guide to In-Kind Donations to LifeROOTS, Inc.

Step 1: Identify the Items to Donate

  • Decide on the type of items you want to donate. In-kind donations to LifeROOTS could include items like office supplies, clothing, hygiene products, non-perishable foods, or equipment that supports people with extra needs.
  • Ensure the items are in good condition and meet LifeROOTS’ needs. Contact LifeROOTS to confirm any specific requirements they may have.

Step 2: Contact LifeROOTS, Inc.

  • Reach out to LifeROOTS to inquire about their current needs for in-kind donations.
  • You can call their main office or visit their website to find more information on the types of items they accept and any guidelines you should follow.

Step 3: Arrange Donation Drop-Off

  • Once you’ve confirmed the items and requirements, coordinate with LifeROOTS to arrange a drop-off time at their location.

Step 4: Prepare a Donation Receipt

  • For tax purposes, request a receipt for your donation. The receipt should list the items donated and their estimated value.
  • LifeROOTS can typically provide this upon receiving your donation.

Step 5: Deliver the Donation

  • Drop off the items at the agreed-upon location.
  • Confirm any additional information needed for tax documentation.

Legacy Gift to LifeROOTS, Inc.

Step 1: Decide on the Type of Legacy Gift

  • Bequest in Your Will: Include LifeROOTS in your will or trust by specifying a gift amount or percentage of your estate.
  • Beneficiary Designation: Name LifeROOTS as a beneficiary on your life insurance policy, retirement account, or bank accounts.
  • Charitable Remainder Trust or Gift Annuity: Consider setting up a trust or annuity that benefits LifeROOTS after providing income to you or loved ones during your lifetime.

Step 2: Consult a Financial Advisor or Attorney

  • Please work with a professional to understand tax benefits and ensure the legacy gift aligns with your financial goals.

Step 3: Specify LifeROOTS, Inc. in Your Documents

Many companies offer corporate matching gift programs, allowing you to double or even triple the impact of your donation to LifeROOTS, Inc. Follow these steps to see if your employer will match your contribution:

Step 1: Check for a Matching Gift Program

  • Explore Company Benefits: Check with your HR department or visit your company’s benefits portal to find out if a corporate matching program is available.
  • Confirm Eligibility: Review any requirements—such as donation minimums, matching percentages, and eligible nonprofits—to ensure LifeROOTS, Inc. qualifies.

Step 2: Make Your Donation to LifeROOTS, Inc.

  • Donate Online: Visit the LifeROOTS, Inc. donation page and complete your donation.
  • Save Your Receipt: Keep a copy of your donation receipt. You’ll need it for the matching process.

Step 3: Submit Your Corporate Match Request

  • Log into the Matching Portal: Many companies use third-party platforms like Benevity, YourCause, or CyberGrants for matching. If your company uses one, register or log in to access matching options. Otherwise, follow your company’s internal matching process.
  • Search for LifeROOTS, Inc.: Use the portal’s search function to find LifeROOTS, Inc. and select it as your match recipient.
  • Enter Donation Details: Provide the date, amount, and any requested proof of donation, such as a receipt.

Step 4: Confirm and Track Your Match

  • Submit the Request: Complete and submit your matching request. You may receive a confirmation email.
  • Follow Up: Processing times vary. Check back in the portal or with your HR department if you don’t see the match reflected after a few weeks.

Volunteer Grant Programs

1. Check Eligibility

  • Confirm with your employer or HR department to see if your company offers volunteer grant programs (Example: Dollars for Doers).

2. Volunteer for a Nonprofit

  • Choose LifeROOTS, Inc. as your nonprofit where you will volunteer your time. We qualify as a nonprofit (501(c)(3) status.

3. Track Your Volunteer Hours

  • Keep a record of the time you spend volunteering. Most programs require a minimum number of hours (e.g., 10 or 20 hours) before you are eligible for the grant.

4. Access Your Employer’s Volunteer Portal

  • Go to your company’s internal volunteer or corporate giving portal (if available). If your company doesn’t have a portal, contact HR for instructions on how to submit hours manually.

5. Submit Your Volunteer Hours

  • Log your volunteer hours through the portal or by completing a paper form (if required). Be sure to include details like:
    • Nonprofit name: LifeROOTS, Inc.
    • Dates and total hours volunteered
    • Description of activities

6. Follow-Up with the Nonprofit

  • Notify us at Marketing@lifeROOTSnm.org so that we are aware of your submission.

7. Receive Your Grant

  • Once approved, your company will donate the grant amount to lifeROOTS, Inc. on your behalf. This can take several weeks, depending on company policies.

8. Stay Updated

  • Check your employer’s guidelines for how often you can apply for grants and any deadlines for submission.

By participating in volunteer grant programs, you’ll make an even bigger impact on your favorite nonprofit organization!

Creating a Facebook Fundraiser for LifeROOTS

Step 1: Log into Your Facebook Account

  • Open the Facebook app or go to facebook.com and log in with your personal credentials.

Step 2: Visit LifeROOTS’ Facebook Page

  • In the Facebook search bar, type “LifeROOTS” or visit LifeROOTS NM’s page.

Step 3: Click on “Fundraisers”

  • Once on the LifeROOTS page, locate the navigation bar just below the cover photo.
  • Click on the “Fundraisers” tab, which might be under a “More” dropdown if it’s not immediately visible.

Step 4: Select “Raise Money”

  • On the Fundraisers page, you’ll see an option to “Raise Money” or “Create Fundraiser.” Click on this.

Step 5: Set Up the Fundraiser

  • Title: Give your fundraiser a compelling title (e.g., “Support LifeROOTS: Empowering Those with Extra Needs”).
  • Goal: Set a fundraising goal amount.
  • End Date: Choose when you want the fundraiser to end.
  • Description: Facebook may auto-fill the description, but you can personalize it by explaining why you are raising money for LifeROOTS and how their work impacts people with extra needs in the community.

Step 6: Add a Cover Photo (Optional)

  • You can either use the default image provided by LifeROOTS or upload your own.

Step 7: Click “Create”

  • Once you’ve reviewed everything, click “Create” to publish the fundraiser.

Step 8: Share and Promote Your Fundraiser

  • Share your fundraiser with friends and family via your timeline, direct messages, or Facebook groups to increase visibility and donations.

George-Smith

Title/Role:
Operations Manager – Commercial Association of REALTORS® New Mexico

Professional Background:
George brings a wealth of experience in operations and administrative management. As Operations Manager for the Commercial Association of REALTORS® New Mexico, he collaborates with the Executive Director and Board of Directors to implement policies and support the association’s strategic goals. President of El Valle Mutual Domestic Water Association

Professional Background:
Edward’s professional journey spans roles as a Medical Sales Manager, CEO with the Water Utility Authority, and working with an insurance company. His leadership extends into multiple sectors, showcasing his versatility and expertise.

  • BA in Health Exercise Performance from the University of New Mexico
  • MS in Physiology from the University of Wisconsin
  • MBA in Hospital Administration Leadership from Southern New Hampshire University

Maggie Silva

Retired

Maggie dedicated over 25 years to the City of Albuquerque’s Therapeutic Recreation Programs and spent more than 3 years as a Special Education teacher with APS. Her long-standing commitment to the community continues to shine through her involvement.

BS in Education from the University of New Mexico

Maggie has a passion for community gardening, enjoying the process of planting, preparing soil, and watching others delight in fresh produce.

Mike Teske

Title/Role:
Independent Agent – Colonial Life

Professional Background:
With 16 years of experience in branch and business banking, Mike has transitioned his passion for supporting families into the insurance industry. His dedication to community involvement is reflected in his active participation in several board appointments and committees.

Olyvia Lee

Professional Background:
Born and raised in Albuquerque, Olyvia Lee has dedicated her career to enhancing community recreation and education initiatives. With extensive experience in coordinator and management roles within the City of Albuquerque, she now oversees the Therapeutic Recreation (TR) youth and adult inclusion programs. Olyvia is passionate about developing training for both full-time and part-time staff to increase awareness of access and inclusion issues for people with disabilities across various programs.

Bachelor of Arts in Signed Language Interpreting with a Minor in Special Education

A creative individual, Olyvia enjoys silversmithing, reading, and staying active in her free time.

Kathleen Barney

Professional Background:
I have over 12 years of experience in the Early Childhood Education field with a diverse background. After earning my BA in Child Development, I began my career at Youth Development Inc (YDI) as a teacher. Over the years, I progressed through roles including Family Advocate, Disabilities Coordinator, and Preschool Center Director. Currently, I serve as the Senior Associate Director, overseeing our home visitation and NM State PreK programs. Alongside my professional journey, I completed my Masters in Early Childhood Education, further enhancing my expertise in the field.

  • BA in Child Development: California State University Los Angeles
  • Master’s Degree in Early Childhood Development: Northern Arizona University

On weekends, if you see me, I’ll probably be riding my bike down a mountain or out on a soccer field. I enjoy staying active, reading books, and discovering new hidden gem restaurants and coffee shops in and around Albuquerque.

Greg Cimino

Professional Background
Greg Cimino has a diverse professional background, having served as a Register of EC Council University, Adjunct Faculty of Brown Mackie College, and Adult Basic Education Tutor at Youth Development Inc. His extensive experience in education and leadership has equipped him with a broad skill set and a deep understanding of human services and resource management.

Definition of Developmental Disability Approved by Centers for Medicare and Medicaid Services (CMS) in the New Mexico Developmental Disability Waiver Application:

The individual must have a developmental disability and mental retardation or a specific related condition. Related conditions are limited to cerebral palsy, autism (including Asperger Syndrome), seizure disorder, chromosomal disorders (e.g., Downs), syndrome disorders, inborn errors of metabolism, and developmental disorders of brain formation.

Developmental disability is defined as a severe chronic disability, other than mental illness that:

  • Is attributable to a mental or physical impairment, including the result of trauma to the brain or a combination of mental and physical impairment.
  • Is manifested before the person reaches the age of twenty-two (22).
  • Is expected to continue indefinitely.
  • Results in a substantial functional limitation in three or more of the following areas of major life activity: self-care, receptive and expressive language, learning, mobility, self-direction, capacity for independent living, and economic self-sufficiency; and
