Customers served (external and internal): all clients, any member of the LifeROOTS Team, board members, all vendors, suppliers and any other members of the public.

Role Summary:  

Functions as ISP developers, advocates, liaisons, crisis managers, and coordinates services for all LifeROOTS service recipients.

Principal Accountabilities: (essential job functions):

  1. Secures funding where appropriate and prepares reports as needed.
  1. Coordinates, schedules, and conducts Individual Service Plans (ISP’s), annual and 6 month reviews.
  1. Conducts intake and orientation on new service recipients.
  1. Assists in transition planning and crisis intervention.
  1. Coordinates/schedules assessments as necessary.
  1. Acts as liaison with all parties involved with service recipients.
  1. Acts as advocate for service recipients.
  1. Assists service recipients in securing services from other programs and agencies.
  1. Processes eligibility for services recipients.
  1. Responds to telephone inquiries regarding records.
  1. Maintains complete records on service recipients as required by LifeROOTS policy, DD Waiver standards and General Funds standards.
  1. Completes all outcome measurement forms.
  1. Participates in training opportunities relevant to job duties.
  1. Participates in Job Development activities.
  1. Assists with the development and implementation of departmental goals and objectives.
  1. Attends all staff meetings.
  1. Responsible for initial training about individuals needs service by LifeROOTS.
  1. Relies on instructions and pre-established guidelines to perform the functions of the job. Primary job functions do not typically require exercising independent judgment.
  1. Responsible for participation in the company’s quality improvement program and ensures that all department-applicable quality improvement requirements are fulfilled.
  1. Responsible for participation in the company’s financial management and ensures that all department-applicable fiscal responsibilities are fulfilled
  1. Responsible for participation in the company’s Health and Safety program and ensure that all department – applicable safety requirements are fulfilled.
  1. Performs other related duties as assigned by the Director of Community Services.

Essential Skills and Experience:

  1. HS Diploma or GED and a minimum of two years of experience working directly with Developmentally Disabled population. BA/BS Sociology, Psychology, or related field preferred.
  2. Computer experience in word processing and spread sheet programs; able to compose reports and letters.
  3. Participation in recent continuing education programs related to Community Services, per licensing requirements.

  2. Time Management: Proven ability to manage one’s own time and the time of others well.
  3. Monitoring: Proven practice of monitoring/assessing performance of oneself, other individuals, or organization to make improvements or take corrective action.
  4. Service Orientation: Actively looking for ways to help people.
  5. Experience working in high performance teams and performing as a proven strong team player.

Conditions of Employment:

  1. Drug Test, Fingerprints and background check.
  2. Must have a valid and current driver’s license with good driving record and must have current auto insurance as required by applicable state law.
  3. Occasionally operates a company vehicle or a personal vehicle while on company business.
  4. Must be at least 18 years of age.

Physical Challenges:

  1. Regular and consistent use of keyboard and mouse.
  2. Must be physically able to perform the essential functions of the position without hazard to themselves or others.
  3. The work is non-sedentary, requiring walking, assisting in lifting and transferring clients, bending, and lifting carrying 50 pounds.
  4. Must have the capacity to toilet and provide personal care with people with disabilities.
